The new world of healthcare is focused on at-risk population groups that require interventions to slow, stop and reverse the progression of chronic diseases. In any particular community, healthcare stakeholders will depend on trusted EIM exercise professionals, as part of their community care team, to meet the demand for engagement in interventions created by the Population Health Management (PHM) care model and Patient-Centered Medical Home (PCMH) approach to coordinated care.
Exercise professionals must be trained to work with patient populations that have been stratified by chronic diseases, levels of risk, motivational readiness and personal preferences. This information provides the initial foundation around which exercise professionals must be able to design and implement effective EIM programs.
The Exercise Professional
EIM exercise professionals are credentialed and continue their training by attending specialty Community Care Team workshops. These workshops are intended to enhance the skills and knowledge for Referrals Engagement, PA Intervention Programs delivery, PA Digital Health applications and other requirements to receive referrals from Healthcare Stakeholders.
The EIM Credential
EIM now includes a credential program that will provide exercise professionals with the opportunity and skills to work closely with the medical community and will provide additional benefits for the EIM credentialed professional. This credential will:
- Be a respected credential for those who work with individuals who are healthy, individuals with health-related conditions who have been cleared by their physicians for exercise (Level 1 or 2), and patients who require clinical support and monitoring (Level 3)
- Supply healthcare providers with a system to validate the qualifications of exercise professionals who are eligible to work with their patients
- Offer an opportunity for exercise professionals to promote their services to healthcare providers and to develop or cultivate patient referrals on a regular basis
