Augusta Health Fitness offers a 10-week monitored medical program based on clinical integration and provider referral. They offer many different “tracks,” from joint prehab to aquatic exercise. The goal if the program is to help individuals regain quality of life and the ability to confidently transition to independent exercise once their program time has ended.
Greenville Health System in partnership with the YMCA of Greenville, the USC School of Medicine Greenville and the American College of Sports Medicine, offers the Exercise is Medicine® program through physicians and healthcare providers to reduce the progression of chronic diseases.
George Mason University’s Freedom Aquatic & Fitness Center
George Mason University’s Freedom Aquatic & Fitness Center is proud to offer the Exercise is Medicine® program for patients who are looking to increase their physical activity level and make sustainable lifestyle changes for improved health. This clinic-to-community care model starts with a health care provider referral and continues the care with individual and group exercise prescription, health and wellness coaching and nutrition education. This program is administered by certified fitness professionals in partnership with the George Mason Kinesiology Department.
Hancock Wellness Center and Health System
Hancock Wellness Center, part of the Hancock Health system, was developed to help our community incorporate not just exercise, but overall wellness into their lifestyles. Hancock Health operates two certified medical fitness centers, in Greenfield and McCordsville, Indiana, with a third opening in New Palestine by early 2021. The centers feature full lines of new Technogym strength and cardio equipment, certified and degreed fitness staff, clinically-integrated fitness programs, physician oversight, nutrition programs, massage, senior fitness programs, youth fitness programs, multiple pools, basketball courts and a variety of group fitness classes.
Healthy Rowan at the Community Care Clinic of Rowan County
Healthy Rowan Coalition, a collaboration of over 25 community organizations, in partnership with the Community Care Clinic of Rowan County, NC offers the Exercise is Medicine® program to reduce the risk of and manage chronic diseases in patients. The clinic serves low income, uninsured adult residents living at or below 200% of the federal poverty level. Through physician prescriptions and referrals to free and low-cost local resources, patients adopt lifestyle changes that could have a positive impact on their health.
Holy Cross Health has partnered with Exercise is Medicine (EIM) to make physical activity assessment and promotion a standard in clinical care. Holy Cross medical providers will refer patients for those at risk for chronic disease to evidence-based exercise programs and qualified exercise professionals. They are committed to the belief that physical activity promotes optimal health and is integral in the prevention and treatment of many medical conditions.
Kirmayer Fitness Center, part of The University of Kansas Medical Center, offers Exercise is Medicine programming to combat acute and chronic diseases. The number one goal of Kirmayer’s medical fitness programming is quality of life improvement through health-related physical fitness.
The LIFT Wellness Center is a Certified Medical Fitness facility in Jackson, TN. Our mission is to serve as an integral part of the health care continuum by using physical activity, nutrition and education to promote healthier lifestyles, prevent disease and improve the overall health and wellness of our members and the West Tennessee Community.
The Ohio State University Wexner Medical Center
The Ohio State University Exercise is Medicine Program is a physician referral program designed to increase physical activity to prevent and reduce complications from chronic disease. Our program is offered at several locations across the greater Columbus area through several community partnerships. All locations provide personalized and safe exercise programming from ACSM certified professionals, providing a vehicle for improving patient’s health, community engagement and access to extended care resources.
The YMCA of Greater Oklahoma City
The YMCA of Greater Oklahoma City is currently partnering with INTEGRIS Health to provide a 16-week Exercise is Medicine® program for employees identified through their annual Health Risk Assessment. The employees are referred through the employee wellness care coordinator. From March 2018, 44 employees have been enrolled across five sessions with a completion rate of 65%.
